AUTHORITY FORM

Should you wish to begin the assessment and eligibility process, we will require your authority to obtain the appropriate information from your previous or current Pension Schemes - click here to download the form and please note that you will need one form for each scheme that you have. The name and address of each Pension Provider should be completed at the top of the form.

If you have already received confirmation, through a prior assessment, that a QROPS is in your best interests and you now wish to proceed to the preliminary stage of the transfer process, then contact us to request a copy of the initial application form.
